What is Home Health Care?

Home Health Care refers to a wide range of health care services that can be given in your home for an illness or injury. At 1^st Meridian Home Health, our skilled nurses, physical therapists, occupational therapists, speech therapists, and social workers provide expert level care through individualized treatment plans.

Even patients with a serious illness can be safely cared for at home at a much lower cost than hospitalization.

Available Services under Physician Supervision

SN Skilled Nursing (RN/LVN) provides nursing care treatment and instructions to the patient and family, coordinating with all others involved in the patient's care. The services can include but are not limited to: dressing change and wound care, catheter care, supervision of medications, family/patient health teaching and counseling, and diabetic teaching.

PT Physical Therapy helps patients regain functional mobility, increase strength and improve range of motion. Therapists provide specialized treatment as prescribed by the physician. The services can include but are not limited to: 1) Therapeutic exercises related to specific loss of function. 2) Utilization of adaptive equipment and aid in rehabilitation.

OT Occupational Therapy increases the patient's independence and ability to perform daily functions by teaching adaptive methods and use of special equipment.
